Dilemma of the Week - Encopresis

I added a new word to my vocabulary today: encopresis. In brief, encopresis can be either voluntary or involuntary bowel movements at an age when developmentally it is expected that children can contain them. Involuntary encopresis has an organic cause such as precipitating constipation. The hard fecal matter was not expelled while constipated, and comes out involuntarily at such a time when the bowel regulates once again. Voluntary encopresis typifies some sort of internal or external emotional struggle and is often found in children with mental health issues such as Oppositional Defiance Disorder (ODD) or Conduct Disorder.

Tonight we had our first tantrum in almost seven weeks – but, thankfully, it did not (or has not yet) escalated into a rage. Our daughter continues to choose to “squeeze in” that last bit of television before going to bed and has chosen to do so over choosing to eliminate in the bathroom. For that choice, she lost television privileges. She has received a consequence around this before, and that was to lose the privilege of watching her very favorite shows – the ones which habitually resulted in the choice to pee and poop wherever she happened to be rather than taking time away to go to the restroom. Previous instances also resulted in my insistence that she learn to do her own laundry when making this choice. So, now she has lost television privileges altogether, so as not to distract her from making the appropriate toileting choice.

True, there is more at work here than just toileting. She does suffer from a psychiatric condition, and it is manifesting itself in this particular arena. To her credit, though there was a lot of huffing and puffing, grousing, insistence that no one in the family loved her (despite my reassurances while meting out the punishment), name calling (though no profanity, this time!), and attempts to lie, she did not escalate into a rage. We are making progress!

What a timely post, I am seriously looking at a 10 yo with this issue, appears to be stress related. On your list, is PTSD also one of the ties?

My grandson had this from 5 through 11, organic, but we never could figure out the exact problem. Stress was always involved. The most memorable event was Matt boggie boarding at the local beach. He was wearing a very tight fitting wet suit, he came out of the water doing the chicken dance and saying 'Number 2 NOW'. Oh did we try. What a cleanup job.

It never seemed to be the end of the world, plans were made including toilets always near by, and spare clothes and handy wipes on hand. I might feel differently if he was a flinger, or one who hides his stuff. It seemed to end by itself. John

As often happens, things come in bunches, and this is the second mention of encopresis I've come across already this morning. If this is some sort of sign,I'm wondering what the rest of the day will be like ...

our boy is 11 next month, we're still fighting this one. nothing we have done so far has made any difference, maybe it will self-resolve like John's grandson? one can only hope! we are truly grateful he is not a flinger. He does all his own cleanup, and purchases his own underpants. still, no dice.

Hi John & thanks! Yes, the formal diagnosis is PTSD. We're seeing the therapist today, so I'll know more then about how it ties in with this diagnoisis, since most of what I read links it to ODD and CD.

Hi Sue, I also thought how thankful I was for the lack of flinging...it's funny the small things one becomes thankful for when parenting children with special issues! :)
